Library Program Proposal

If we are unable to use your proposal at this time we will keep your information for future consideration. Due to the volume, the Library is not able to respond to all proposals. If we are interested in your program, we will contact you.

Accessibility Regulations for Programs

The Accessibility for Ontarians with Disabilities Act requires that presenters be trained in accessible customer service, information and communications and in the Ontario Human Rights Code as it relates to disability. Presenters may need to make accommodations in programs they offer through the Library.

Use of Personal Information

Personal information on this form is collected under the authority of the Public Libraries Act and the Municipal Freedom of Information and Protection of Privacy Act. It will be used to plan and evaluate program proposals in compliance with federal, provincial and municipal legislation and Whitby Public Library Rules of Conduct, Privacy Policy and any other applicable library policies.
